Flu Boosters

If your child received a first-time flu vaccine this year, a booster dose is needed. Children under the age of 9 years who have never had a flu vaccine before require a booster dose at least 28 days after the first injection. Flu boosters are available at your scheduled appointment(s).
Starting November 1st, flu boosters will be available without an appointment Monday through Thursday evening from 5:00 p.m. to 7:00 p.m., and Sunday morning from 8:30 a.m. to 12:00 Noon.
Please respect the clinic time allowance.
Help avoid delays:
- Educate yourself and your family by visiting the CDC website - http://www.cdc.gov/flu/ - for information regarding the 2011-2012 flu season recommendations.
- Bring your current insurance information.
- Read the Vaccine Information Sheet. Click here for Flu Vaccine Information Statement (VIS)
- Print, complete and bring with you the Influenza Screening Form/Advance Beneficiary Notice for each child you plan to have vaccinated. Click here for Influenza Screening Form/Advanced Beneficiary Notice (ABN)
- Flu clinics are for flu vaccines only. If you think your child may be ill or you have questions regarding the flu vaccine, please call our office to speak with a nurse. A physician will not be available at flu clinics.
All of our flu vaccine is preservative free.
FLU VACCINES WILL BE ADMINISTERED AND CLINICS WILL BE HELD AS LONG AS VACCINE PRODUCT IS AVAILABLE. PLEASE CHECK THE WEBSITE FOR UPDATES, CHANGES AND/OR CANCELLATIONS. UP-TO-DATE INFORMATION WILL ALSO BE AVAILABLE ON OUR FLU INFORMATION LINE AT 565-9030 EXT 622.
