HOW TO REGISTER FOR FLU VACCINATIONS

Our office is currently booking appointments for both seasonal and H1N1 flu vaccinations.   We ask your cooperation in following the guidelines below so that we can process these requests and get your children vaccinated as quickly as possible.  

All requests for appointments must be done through the on-line registration form.  This will help us track product availability.  A staff member will contact you at the telephone number you provide on the request.  Please be sure the information given is up-to-date.  Due to the large volume of requests, we will not be calling families multiple times. 

To assist us in the registration and vaccination process, please go to the "Flu Screening Form and Information Sheets" to review the products available and make your selection on the registration form based on these recommendations.   Please download the screening form as you will need to bring the completed  form to your scheduled appointment.  This must be completed before you can receive your vaccination.   

The vaccination registration form can be accessed by going to the "For Parents" section.  Go to the the blue box and select "Patient Forms".  From Patient Forms, select "Flu Clinic Request".  You will be taken to "Terms of Use".  Select "Flu Clinic Request".  After reviewing the Terms of Use, you must select "Agree".  This form is for established patients of Suburban Pediatrics only.  Please complete all required fields.  Again, be sure the information you provide is accurate; it is the only information we will be using to contact you.  When complete, select "Submit Flu Clinic Request".

We will have staff members calling to schedule appointments as long as we have product available.  As more product is shipped to us, we will open more appointments.  PLEASE BE PATIENT.  DO NOT SUBMIT MULTIPLE REQUESTS.  IT MAY TAKE US A FEW DAYS TO GET THROUGH TO YOU.
